Referring a Patient:

Please send us your patient's demographics, insurance information and a signed prescription to our South (719-634-1112) or North (719-282-2330) locations, email us at admin@synergympt.com or simply send the information with the patient.

Insurance Benefit Verification and Pre-authorization:

We provide the patient with a detailed explanation of benefits and even take care of the pre-authorizations when applicable.
